All students enrolled in a Catholic school in the State of Texas must be fully immunized in accordance with the immunization schedule adopted by the Texas Department of State Health Services.

A completed immunization record upload is required to be enrolled at St. Joseph Catholic Academy.

The only exemption to the foregoing requirement is a medical exemption signed by a licensed physician (MD or DO) authorized to practice in the State of Texas.
